The Physician Group of Capital Health Plan (PGCHP) is focused solely on taking care of Capital Health Plan (CHP) members. Whether you are a long-time patient or new to their practice, every aspect of your patient-centered medical care is coordinated by a primary care physician dedicated to improving your health.

The Physician Group of Capital Health Plan provides evidenced-based comprehensive health care for our patients of all ages, genders, and conditions. Our goal is to promote disease prevention and healthy living, so in addition to diagnosing and treating illnesses, we also provide preventive care, health risk assessments, immunizations, screening tests, comprehensive management of chronic conditions, and education on maintaining a healthy lifestyle.

In addition, the Physician Group of Capital Health Plan utilizes an extensive network of speciality care physicians located throughout our service area who provide care to our members in their own offices. CHP’s network also includes hospitals and other clinical facilities throughout the service area, and Centers of Excellence when medically appropriate.

One of the most important health care decisions a patient will ever make is selecting a Primary Care Physician (PCP). At the heart of a Patient Centered Medical Home is your ongoing relationship with a PCP and the Care Team. Your PCP at the Physician Group of Capital Health Plan will provide comprehensive health services through an integrated health care delivery system. Evidence-based medicine and clinical decision-support tools guide decision making. You are at the center of the medical team and actively participated in the decisions concerning your health.

Your immediate Care Team includes your PCP, nursing staff, and reception staff. This team is your health care partner and central coordination point for everything pertaining to your good health. Everything is coordinated across all elements of the complex health care system and communication is streamlined through the online secure patient portal.

Colon Screening Program

Colorectal cancer is a cancer of the large intestine or rectum, and it is the second leading cause of cancer deaths in the United States. The primary goals of colon screening are to prevent cancer from occurring in the first place, if possible, and to detect colon cancer early, when there are better and more effective treatment options. The American Cancer Society and the U.S. Preventive Services Task Force (USPSTF) recommend that everyone should start colon cancer screening at age 45. Everyone should continue screening through age 75, and, in some cases, screening should continue through age 85. Some people are at increased risk for colon cancer due to their family history and should speak with their primary care doctor to ask if screening should start before age 45.

The easiest way to participate in colon screening is by completing a stool kit (commonly called a “FIT kit”). The kit is easy to do and can be completed at home and then be returned to the lab by mail, or it can be delivered to one of the Capital Health Plan (CHP) Health Centers. For the FIT kit, only one small sample of stool is required, and there are no dietary or medication restrictions required.

Another option for screening, which is considered the “gold standard”, is a colonoscopy. The benefit of having a colonoscopy, compared to other screening methods, is that precancerous growths can be detected and painlessly removed during the exam, before cancer cells develop, therefore preventing colon cancer from occurring.

Many people delay scheduling a colonoscopy because they have been told the exam has to be done with IV sedation and, given people’s busy schedules and personal responsibilities, they can’t be “out of it” for a day. And, many people simply don’t want the risks associated of IV sedation. In 2011, the CHP Colon Screening Program began offering in-office colonoscopies which are done at CHP’s Centerville location. There, colonoscopies are successfully done without IV sedation, and the exam is very well tolerated and people can drive themselves to and from the appointment and resume their usual responsibilities immediately after the procedure. If you are a healthy adult and have no worrisome symptoms (e.g., unexplained abdominal pain, rectal bleeding), you can schedule an appointment for your colonoscopy by calling the Capital Health Plan Colon Screening Program (850-523-7350).

There are actually several options for participation in colon cancer screening so if you’re not sure which screening option is right for you, please contact your primary care physician to have an important discussion about periodic screening, the various options available, and the role screening plays in disease prevention.

CHP Infusion Services

Infusion Therapy deals with all aspects of fluid and medication administration, through a vein in the arm or directly into the skin. Special infusion pumps can be used for this purpose. Advantages of coming to CHP’s Infusion Center include ease of access for you and your provider, a comfortable setting, and a friendly staff.

Wound Care Center

Your skin is very important to you and to your overall health. Capital Health Plan is pleased to offer our members options for the treatment of skin and wound conditions. The Wound Care Center offers patients options for treatment of most skin conditions as well as most wound care services.
